Consent for Screening Ultrasound Services, Privacy Practices & Terms and Conditions

Patient information is handled in accordance with applicable federal and state privacy laws and the facility’s privacy practices. Patient records, images, and reports may be stored electronically and shared with interpreting providers, physicians, or authorized healthcare entities as necessary for service-related purposes.

Nature of Screening Services

The ultrasound examinations provided are screening services intended for preventative and informational purposes only. These examinations are not intended to diagnose, treat, cure, or prevent any disease or medical condition and are not a substitute for a comprehensive medical evaluation, diagnostic imaging study, or consultation with a licensed physician or healthcare provider.

Screening Limitations

While every reasonable effort is made to obtain accurate images and interpretations, no screening examination can detect all abnormalities or medical conditions. Ultrasound technology has inherent limitations, and false-positive, false-negative, or inconclusive findings may occur. A normal screening result does not guarantee the absence of disease or abnormality.

Interpretation and Recommendations

Screening results and reports are based solely on the images obtained during the examination. Additional diagnostic imaging, laboratory testing, physician consultation, biopsy, or other medical evaluation may be recommended depending on the findings.

Follow-Up Responsibility

The patient understands and agrees that they are solely responsible for obtaining appropriate medical follow-up and sharing screening results with their physician or healthcare provider. Failure to seek timely medical evaluation may result in delayed diagnosis or treatment.

Emergency Disclaimer

These screening examinations are not intended for emergency medical conditions or urgent medical concerns. Patients experiencing severe pain, acute symptoms, medical emergencies, or concerning changes in their condition should immediately contact their physician, seek emergency medical care, or call 911.

No Physician-Patient Relationship

Use of these screening services does not establish an ongoing physician-patient relationship with the facility, ultrasound technologist, interpreting radiologist, medical director, or affiliated providers.

Electronic Communication Consent

By providing an email address, telephone number, or other electronic contact information, the patient consents to receiving reports, scheduling information, reminders, invoices, educational materials, and other communications electronically, including by email and/or text message where applicable. While reasonable safeguards are used to protect personal information, electronic communications may carry inherent privacy and security risks.

Consent to Examination

The patient voluntarily consents to the ultrasound examination and acknowledges that the procedure is non-invasive and generally considered low risk. The patient affirms that all information provided is accurate and complete to the best of their knowledge.

Incidental Findings

The patient understands that incidental or unexpected findings unrelated to the purpose of the screening may be identified. Recommendations for additional evaluation may be made based on such findings.

Results Delivery

Reports are generally delivered within the estimated turnaround time provided at the time of service; however, delays may occasionally occur due to technical, operational, or physician review requirements.

Financial Responsibility

The patient understands that screening services may not be covered by insurance and agrees to remain financially responsible for all fees associated with the services provided unless otherwise stated in writing.

Cancellation and Rescheduling

Appointments canceled or rescheduled without adequate notice may be subject to cancellation fees or forfeiture of deposits, if applicable.

Release of Liability

To the fullest extent permitted by law, the patient releases and holds harmless the facility, staff, technologists, interpreting physicians, contractors, and affiliates from claims arising from limitations inherent to screening examinations, delayed follow-up, failure to seek medical care, or interpretation limitations, except in cases of gross negligence or willful misconduct.

Modification and Termination: RadiantViewâ„¢ reserves the right to modify or terminate any aspect of the services provided, including these Terms, at any time and without prior notice. We may also refuse or restrict access to our services for any reason, including but not limited to violations of these Terms or applicable laws.

Intellectual Property: All content and materials provided by RadiantView Wellness Ultrasound including but not limited to text, images, logos, and trademarks, are protected by intellectual property rights and remain the sole property of RadiantViewâ„¢. You may not use, reproduce, modify, or distribute any of our intellectual property without our prior written consent.
